Simple-survey is out.
Three options
1) "survey" package in CVS openacs-4/packages/survey
2) "complex-survey" package in CVS openacs-4/contrib/packages/survey (allows sections, branching..)
3) assessment, like complex survey in CVS openacs-4/packages/assessment
Assessment is the one that is being maintained most at the moment.
