Answer to question 2:

1) Log in as a sitewide admin on one browser and go to /ds.
2) On another browser which is not sharing cookies, log in as a regular user and reproduce the problem.
3) refresh the screen on the first browser.  The long query will be sitting on the list; click on it to reveal the guilty database query.
