I found good documentation about ad_form but how do you display it?  What is the relation of ad_form in tcl page to adp page?  Does anyone have a SIMPLE and COMPLETE sample that WORKS?

You need to put <formtemplate id="form_name" /> in your adp-file where you want the form to appear.
