I cut the two lines from above and pasted them into a csv file. I then tried bulk uploading into my .LRN instance and it worked fine.
The most likely possibility is there is some subtle mis-formatting of the actual file you are trying to bulk-upload. If you are willing, email me the file and I'll try uploading it into my site and see what happens.
