Brian, on your "wizard" problem, I have no idea, I've never used that stuff.
In your Request Processor log above, the "Applied transformation from /web/aimsprotoff/www / forms/admin/question-add" stuff is surprising and mysterious to me, but then I'm not familiar with the guts of the Request Processor. So that behavior might be completely normal and have nothing to do with your wizard problem.
