Regarding templates for surveys:
My plan is to fix the question rendering code to be fully templated using the form builder or ad_form.
Then create form templates that can be selected by the admin. Currently there are 3 display styles. Those three will be rewritten as templates and more can be easily added.
I'll take a look at the data model changes soon.
Matthew: did you look at simple-survey, it has unused code and tables for scored surveys which might be of assistance.
The supporting code was all commented out, and it wasn't needed for Sloan so we did not add it in to Survey.
